There is no fixed interval — it depends on exposure. A wall that takes direct afternoon sun and driving rain will chalk and fade faster than a sheltered elevation on the same house. Fading, chalky residue on your hand, and algae growth are the practical signs it is due.
Because most exterior coating failures are bond failures. Algae, chalk and dust form a layer between the wall and the new paint. The coating sticks to that layer instead of the wall, and it lifts in sheets a year later. Washing is the least visible part of the job and one of the most important.
Not reliably. Exterior paint sheds rain, but it is not a waterproofing system. If you already have damp appearing inside after rain, that needs a waterproofing treatment on the affected area first — painting over it will hide the evidence, not the problem.
It depends on the height and the elevation. Reachable heights can be done with ladders and access equipment included in the rate. Taller or more complex elevations need proper scaffolding, and where that applies we quote it separately rather than working from an unsafe position.
